Release of Medical Records

If you are transferring to the care of another practice, you will need to notify our office in writing to request a release of your medical records. Our office has contracted with Smart Corporation scanning service to scan and transfer your medical records. All fees are based on rates allowed by the State of Virginia, Virginia State Rates, Virginia Code Ann. 8.01-413. Base fee is $10.00 plus $.50 for each page, for the first 50 pages, and $0.25 for each page after 50. Smart Corporation will invoice you directly.

• Individual Rate: scan standard 2 years of medical history and third year assessment, maximum fee per chart $23.00
• Family Rate: scan 2 years of medical history and third year assessment for each chart, maximum fee $35.00
• Complete File: Scan entire medical chart or dates requested; this applies for all requests greater than the 2 years standard medical history. Virginia State Rate applies, starting with page 1. The complete file is mailed to the patient.

A separate signature is required for release of HIV/drug/alcohol/psychiatric information.
